Tax Accountants for Doctors

Tax Accountants for Doctors

Your tax sorted, whatever your working pattern.

Most doctors are juggling NHS pay, locum income, private fees and a pension that throws up annual allowance charges without warning. An accountant who already knows how medical income works saves you the explanation. Fixed fees, FCMA-qualified, same-day replies.

  • Self-assessment filed accurately — multiple income sources handled correctly
  • NHS pension annual allowance charges identified and planned for in advance
  • Locum limited company accounts prepared and Corporation Tax filed on time
  • Private practice expenses claimed properly — GMC fees, indemnity, CPD and more

No long-term contract. If it is not working after three months, you leave with clean books and nothing owed.

Sound familiar?

Doctors have complicated tax. Most accountants do not.

NHS salary, out-of-hours sessions, locum work, private fees — your income rarely comes from a single source, and most general-practice accountants treat it as if it does. Annual allowance charges appear with little warning. Expenses go unclaimed because no one pushed. Self-assessment gets filed late, or filed incorrectly, or both.

  • Annual allowance charge arrived as a surprise because nobody was monitoring your pension growth
  • Locum income going through a personal account when a limited company would be more tax-efficient
  • Legitimate expenses — GMC registration, medical indemnity, CPD — never properly claimed

What properly sorted looks like

An accountant who already knows how doctors are paid does not need the explanation — they already know the questions to ask. Your tax position is visible throughout the year, not discovered in January.

  • NHS pension growth monitored and annual allowance charges flagged well before year-end
  • Locum and private income structured tax-efficiently from the start — limited company where it makes sense
  • Every allowable expense identified and claimed — nothing left on the table
  • Fixed monthly fee — no surprise bills and no hourly rate ticking while you ask questions

What clients say about working with us

Doctors and professionals with complex tax situations tend to stay. Several of our clients have been with us for seven, ten and eleven years.

★★★★★

Unlike my previous Accountants, Niall is far more hands-on, available for advice and able to care for me as an individual, rather than a faceless client. His fees are very reasonable and the money he has saved me more than covers them.

What you get

Everything a doctor's tax position needs

One engagement covers the full picture — personal tax, practice accounts, pension planning and compliance — delivered by a chartered firm via cloud accounting software.

01

Personal Tax and Self-Assessment

Your self-assessment return covers every income source — NHS pay, locum earnings, private fees, investment income and any rental income — filed accurately and on time. Tax-efficient structuring is considered from the outset, not as an afterthought. You will know your liability well before the January deadline.

Included as standard
02

Locum and Private Practice Accounts

Whether you operate as a sole trader or through a limited company, your accounts and Corporation Tax return are prepared and filed correctly. The question of whether a medical limited company makes sense for your circumstances is one we can model before you commit. Bookkeeping is handled through cloud software so your records are always current.

Limited company or sole trader
03

NHS Pension and Annual Allowance

The NHS pension annual allowance charge catches doctors out regularly — particularly those with career average revalued earnings or those returning from reduced-hours periods. We monitor your position and flag exposure early, so the charge does not land as a surprise in your self-assessment. Scheme Pays elections are handled where relevant.

Why OD Accountants

Why doctors work with us, and stay

Three things that make a practical difference when your tax situation is more involved than most.

🩺

No Explaining How Medical Income Works

NHS pay, locum agencies, private billing, Performers List payments — the way doctors receive income is not straightforward, and most accountants require a long briefing before they can help. We already know the structure. You will not spend the first hour explaining how you get paid.

📊

Pension Charges Caught Before They Land

The NHS annual allowance charge is one of the more costly surprises in personal tax. It is also largely avoidable when someone is monitoring your pension input figure during the year rather than discovering the problem in January. We track the position and flag it early enough to act.

🏛️

Chartered Qualification, Not Just Compliance

Founder Niall O'Driscoll is an FCMA, CGMA — a Fellow of the Chartered Institute of Management Accountants with over a decade of commercial and restructuring work behind him. For doctors considering private practice, a limited company, or a significant change in working pattern, that depth of financial judgement matters.

Getting started

Up and running in four straightforward steps

Most clients are fully onboarded within a couple of weeks. The switching process is handled by us — your previous accountant needs to do very little.

1

Book a Discovery Call

A short call — usually 20 to 30 minutes — to understand your working pattern, income sources and current tax position. No preparation required on your part. You will leave the call knowing exactly what the engagement covers and what it costs.

2

Receive a Fixed-Fee Proposal

You receive a clear written proposal covering everything in scope and the monthly fee. No ambiguity about what happens if you have a question mid-year. No hourly rate surprises.

3

We Handle the Switch

If you are moving from another accountant, we contact them directly for the professional clearance and transfer of records. You do not need to manage that conversation. Your cloud accounting access is set up and your historical data is in place.

4

Your Tax Position Is Visible Year-Round

Your accounts are current, your pension exposure is monitored and your self-assessment is prepared well before deadline — not filed in a rush in January. Which is, frankly, the way it should be.

Questions

Things doctors usually ask us first

Do you understand the NHS pension annual allowance charge and how it affects doctors?+

Yes. The annual allowance charge is a recurring issue for consultants, GPs and senior doctors in the career average scheme — particularly those with significant pensionable pay growth. We monitor the position during the year rather than discovering the liability at the point your self-assessment is prepared. Scheme Pays elections are handled where they are the right mechanism.

What does the engagement cost and what does the fee include?+

Fees are fixed and agreed before anything starts — no hourly rates and no charges for asking questions during the year. The scope covers everything relevant to your situation: self-assessment, accounts preparation, Corporation Tax where applicable, and proactive tax advice. Exact pricing depends on the complexity of your income sources; we will give you a clear figure after the discovery call.

My books are behind and my last accountant was not very proactive. Is that a problem?+

It is not unusual. Most clients who switch to us arrive with records in some state of disarray — late filings, incomplete bookkeeping, or simply nothing done since the last year-end. We handle the catch-up work and bring everything current. There is usually a one-off catch-up fee for historic work, which we agree upfront before starting.

Is there a long-term contract or minimum term?+

No long-term contract. Engagements run on a rolling monthly basis. If it is not working, you give notice and leave with clean, complete records. We are not in the business of making switching away difficult.

Should I operate through a limited company for my locum or private work?+

It depends on your earnings, your other income sources, and how you intend to draw money from the business. A medical limited company can offer tax advantages for higher earners with locum or private income, but it also introduces compliance obligations and is not the right structure for everyone. We will model both scenarios before you make the decision.

Can you help me understand what expenses I am entitled to claim as a doctor?+

Yes. GMC registration fees, medical indemnity premiums, CPD courses, professional subscriptions, home-working costs for administrative work, and equipment used in private practice are among the most commonly missed deductions. We review your position as part of onboarding and make sure nothing allowable is left unclaimed going forward.
